Try starting from scratch - from the start. OAS 4.0 is not very good at changes or adding components without adversely affecting what is already there. Try installing everything from the beginning including the samples and you should find it works OK.

Note that the application status is always DOWN until it is first accessed, then it loads and runs. It will time-out and unload after a few minutes not being accessed and be ready to reload when accessed again.
