Problem 1.
The forms cartridge is a C Web Cartridge, nothing else. Documentation error.

Problem 2.
In OAS 4.X.X.X this option is not included, i.e there is no such way to bind a specific listener to a cartridge. This option was included in WAS3.0 and below.
Documentation error.

"Can not service this request, please try again later" This depends on wrong url call. Can depend on numerous things. Is the right virtual path called?
After adding a new cartridge and application, restart OAS. Sometimes it helps.
