Can I change the database structure (adding fields in the tables) on the fly?

We plan to develop a base database right now. However, most likely, the requirements will be changed from time to time. The main changes will be extra fields.

I think it is possible to do so.
What is the best way to implement this?

Thanks a lot for your help.
