The entities in a database are exactly those needed by the application. Design your app, and you'll know what entities you need. Don't design your app, and it won't matter any way. In that case you can use the following:
CREATE TABLE MY_HOMEWORK
(ID_NUM CHAR(5) PRIMARY KEY, STREET_ADDRESS CHAR(25), QTY_ON_HAND INTEGER, ANNUAL_SALARY INTEGER, OFFLINE_DATE DATE, STATUS_FLAG_1 CHAR(1), FATHERS_DOB DATE, VENDOR_CODE CHAR(4), AMOUNT_DUE NUMBER(5,2))
