There are a number of applications to do this - Oracle's Developer/2000 is the 1 I've used and found it pretty straight forward. There are also a number of 3rd party apps to do it or you can contract out to someone to do it for you - it will depend on your particular situation and resources ( some can be pretty expensive for just a small number of forms). The docs in Dev/2000 has all the info you need (basically you fill out a form and press ther GO button).
