I think I see what you mean: if extents can't be reused because they contain active transactions, then how is it possible that multiple transactions use the same extent in the first place!

The difference is this: when a transaction gets access to an extent that is used by other transactions and thus share the extent with this new transaction, the extent is *in use* . The problem with not getting access to extents which contain active transactions only arises when *starting* to use a new extent.
