UTL_FILE error without exception.

Tryint to produce a file and get an unusual fault. The file creates ok, but on writing produces an error which is not an exception. If I create the file myself, grant full writes to it and open in append mode the same thing happens. This suggests that is not an access mode or rpivelege problem. Any ideas?
