Could your problem just be O/S permissions?

AFAIR, you can only write to the directory that matches utl_file_dir. An * here meaning any directory, as long as the O/S permits writes to the named location.

/usr/mark and /usr/joe both match * but does oracle have permissions
to write here too?

Don't just consider the permissions of the named file e.g.
/usr/mark/out-file; this file may be writable by the 'world' but the
host directory (/usr/mark) may not.

If you don't have permission to write to the directory you may not be able to write to a file in that dircectory.
