i'm not sure whether i got exactly the same error but sometimes after a crash of oracle but not the system there are shared memory segments and semaphore sets left orphaned (at least on SGI). this seams to confuse oracle totally. just get rid of them with ipcrm unix command; instance recovery works fine then.
