On HP-UX we got this problem when the definition of shared memory segment remains on disk.
1° we stop the instance with ABORT option. 2° under the directory $ORACLE_HOME/dbs you shouldn't find a file called "sgadef[$ORACLE_SID].dbf"
3° If this file exists, remove it.
4° restart the instance.
