At last - someone else what has seen this problem. I struggled with this problem for several days with little help from Oracle.

Basically what caused this problem (at least for me) was that the Concurrent Manager was firing off additional services which weren't getting shutdown when the Concurrent Manager was shutdown. Because these additional services were still running, the Concurrent Manager would not start up (perhaps it thought it was already up and running).

Once this happened there were only two ways to fix it: reboot the server, or bring up the Task Manager and kill each process manually. I you do the latter, be VERY careful that you don't kill the wrong process. If the process is running from the \FND\BIN directory, you will be ok.

There are only two ways I found to shut down the Concurrent Manager that would shut down every thing clean: via the SERVICES applet in the Control Panel, or via a command line using the CONSUB program. (I'm not sure if I have the name exactly right since I am at home). If you use the CONSUB program DO NOT do a Shutdown Abort.
