HELP: ora-01536 : space quota exceeded for tablespace "INDEX"

I get this error when a user try to insert a record on a specific table (authorization table) even though the oracle user has UNLIMITED space quota.

If the user insert a different record in another major table , we are not getting this problem. Only with that particular table (Authorization).

I've checked space on the tablespace to find a plenty of it (1/2 full). I doubled checked with dba_ts_quotas to find the user already been granted Unlimited (-1).
