Yes, the cat* scripts probably weren't run. That's ok in one sense, but you may be able to infer that things are in an undefined state wrt oracle versioning. I would suggest looking at the patch instructions at the oracle patch site, and tell the customer he shouldn't fall behind on versions. Scripts are in $ORACLE_HOME/rdbms/admin.

Somebody goofed setting the default tablespaces when creating the schema. If there was an application script run to create this, the vendor is a dork. Follow the directions others have given you about tablespaces.
