Adding instances to listener

We are running version 7.3.3.0.0 on Solaris 2.5.1 with version 2.3.3.0.0 of the Listener. When we add a new database to the server I have been doing a LSNRCTL RELOAD after copying the new LISTENER.ORA & TNSNAMES.ORA files. Everything seems to be ok and you can connect to the new database. However, any new connections to instances using the multi-threaded server no longer use it. All new attaches start up their own process. If I shutdown everything and restart the listener everything works fine. However, this is on the production server and it is very hard to schedule even a brief downtime.

My question is should the reload work? If not what should I be doing to add new instances to the listener.
