Index Rebuilds

I am looking for some general rules on when it is necessary to rebuild an index. I have heard that when the ratio of del_lf_rows to lf_rows > 20% you should rebuild the index. Also when the ratio of del_lf_rows_len to lf_rows_len > 1%, then rebuild.
Does anyone agree or disagree with these general rules. Also, when does pct_used become important.
