Webserver 2 and Des2k Error

I'm getting the following error message written to the owa_dba.err logfile when I attempt to execute a very simple live HTML query form that's been generated by Designer/2000.

I'm running 7.3.2.1.1 on NT 4.0 with Webserver 2.0.1.0.1 with an application
generated by 1.3 of Designer 2000.
