SQL*Loader not inserting all data

I have a SQL*Loader control file that contains the control information
as well as the data.  There are about 12M rows of data yet only about
2M them make it into the database.  I checked the BAD file that I
specify in the control file and it only has a couple of rows in it.  I
am using the DIRECT=true flag in an attempt to speed the insert of
data.  Any idea what I might be doing wrong?
