ACCUM - how does it work?

Can anybody tell me how the ACCUM within the use of the ORACLE context engine is implemented in terms of the search strategy? What I am interested in is, how the combination of results from several terms is realised. Is there something like a two-step process performing an AND combination first and an OR combination next when it comes to rank and order the hits?
