Most of the big name Firewall vendors e.g., Check Point and Raptor/Axent/Symantec, I believe, have a built-in application proxy to enable this through their firewall products. You didn't mention what operating system you have a preference with. If Windws, Microsoft has a product called the ISA Server which is pretty flexible. This isn't a bad product.
