Reports 3.0 & DD Form 1149

I have to write a report in the format of the standard Department of
Defense Form 1149 (Requisition and Invoice/Shipping Document).  Would
anybody by chance already have created this form in Reports 3.0 or 2.5
and be able to email me the .rdf file?  Appreciate the time savings
this would be for me!
