I have also experienced "lost sequence numbers" if the buffer option on the sequence is set to; eg "20"; because then Oracle will claim the next 20 sequence numbers from the sequence generator for "this" session, and use from those until there are no more left.

_If_ users log off before the sequence numbers have been used, there will become a gap in your overall-sequence, because the next session will pick numbers from 21 and to 40.
