Oracle and NT SP5 do not play well together. I am not exactly sure as to the technical reasons, but Oracle will barely support anything on NT SP5. Try backing out the service pack to SP4 or upgrade to SP6 and see what happens. This is just a suggestion, but it is the first thing that pops to my mind to try. I hope this helps.
