Authorization errors with OAS 4.0.7

I'm having an intermittent problem with Netscape and IE browsers authenticating to a site using OAS 4.0.7. Some users can never get in, some always get in with no problems. Some can use the forward and back buttons in their browser a few times and then they are allowed in. The ID and password is being typed correctly in all cases. Any help would be greatly appreciated.
