NT SP5 is more stable than SP4. I would recommend skipping from SP3 to SP5.

We experienced some random Oracle disconnects under SP4, but these went away under SP5.

If you have SP4 Oracle works better if SP4 is applied and then Oracle is installed.

Another caveat: never apply an NT service pack while your Oracle instance is running. I would recommend shutting down Oracle, put the service on manual start up, reboot so your server comes up cleanly without Oracle, apply the service pack, reboot, reset the Oracle services to automatic start up, and reboot one more time. We have had service packs do strange things to Oracle when system administrators have left it running while applying a service pack. Also, always create an uninstall directory when applying a service pack. If it doesn't work, uninstall it.

I have never seen an instance simply "stop working" because of a service pack. Typically, something else has gone wrong with the machine or the Oralce instance that can be repaired without removing the service pack or reinstalling Oracle.
