SQL *Menu 50 to forms4.5 upgrade.....frm 10043 error

Hi all,
I am trying to migrate SQL *Menus5.0 to a Forms 4.5 module. But the problem is that when i try to use the forms generator(f45gen32) As shown in the Forms reference manual Appendix A I used the forms generator as follows:
fgen32 menuname id/pwd module_type=menu upgrade=yes. But when i do this it gives me an error FRM-10043 saying unable to open file. The menu files that i have are of .sql and .dmm extensions.
Can anyone guide me through the proscess of conversion. any help is appreciated.
