SQL-2103 - Inconsistent Cursor Cache - WHY???

Does anyone know specifically what might cause an SQL-2103 error? I know it could be an invalid pointer or somesuch, but what would cause the pointer to become invalid?

It seems to happen with implicit cursors, but it's been happening on things as simple as select a value from a sequence.

Any help would be greatly appreciated.
