I am curious as to where the actual access violation is. Do you know what DLL is involved?

There is a similar problem with SQL Worksheet on NT4SP3, but apparently only after Outlook is installed. In that case the access violation is in GAPI.DLL. Others have reported similar problems, but with slightly different symptoms. I expect that the problem is in with some Microsoft DLL that was changed subsequent to it's use by Oracle.
