Have you set the UTL_FILE_DIR init.ora parameter?

This dictates which directories you will be allowed to write to and can occur many times in the init.ora file for multiple directories. You can set it to * for all directories, however this is not advised as the user executing UTL_FILE has the oracle owners file permissions (due to the setuid of the oracle executable) which may be higher than required.
