This sounds like the same problem I've had. If your generating a form <FORM> with a submit button and the Submit button doesn't have NAME="action" on the button it won't work.

if you exclude the NAME="action" you get the wrb.exe application error and must reboot the NT server.

if you include NAME="anythingelse" you get a server error and the web broker is still running.
