I have seen Forms 3.0 issue this message, 'unable to allocate record buffer' when running on UNIX system under Oracle version 7.1.6 and it was due to the '/var' filesystem being 100% full.
You should probably look to see where Oracle Forms takes his temporary work space from at the OS level. You may have a full filesystem somewhere.
