I dealt with this situation recently. I even tried reorganizing LOBs but no avail. In the end I’d to sought help from oracle support who dived in and suggested few internal memory parameters to be set and then reorganize LOBs once again. It has helped somewhat but to what extent is still too early to call.

alter system set "_enable_space_preallocation= 3;

alter system set "_ktslj_segext_warning" = 0;

alter system set "_securefiles_concurrency_estimate"=50 scope=spfile;

I had one application that stores lots of transactional data in LOB and the same is updated heavily. Does frequent update on LOB

1. Impact system performance heavily
2. Can cause lots of Fragmentation
3. Need any frequent maintenance as part of optimization

The database is 18c and LOBS are secure files and Partitioned.
