My understanding is that Oracle releases a new OVJM PSU every quarter. These are typically compiled against a specific PSU, so yes, you will need to apply the new OJVM PSU with every PSU application.

Oracle releases a standalone DB/GI PSU and standalone OJVM PSU, or there is a combo patch that includes both. When using the combo patch, you still need to apply the PSU and OJVM PSU with separate opatch commands. Keep in mind that Oracle does not currently support rolling or standby first patching for the OJVM PSU. Also, the post patch scripts require that the database be started in upgrade mode, which means cluster_database must be set to false for a RAC database.

There is some pretty good information on the mitigation patch (which allows you to apply the patch in a rolling fashion, but kills Java usage in the database) in MOS note #1929745.1.
