Once, many years ago, I had a similar problem. We imported a database exp dump file from a German customer, into one with an ASCII 7 character set.

All the accented German characters got de - accented and caused duplicate rows with corresponding Primary Key constraint violations.

The problem vanished when we created a database that matched character sets.
