There are a variety of reasons on old archive recovery point is selected, some are bugs and some are human error.
  1. Is a table space or data file in backup mode?
  2. Was a tableapace or datafile added at the archive log recovery point it is looking for?

Yes, the recovery catalog is in place. I'm at home now, so I cannot tell you which version ... we are trying to run a recovery out of rhe control files now. If that doesn't work, we'll have to open a ticket with Oracle <SIGH />

Can we assume Recovery Catalog is in place? If yes, which version? was it updated when they went to 10.2.0.4?

Steps to determine are covered in http://docs.oracle.com/cd/B28359_01/backup.111/b28273/compat002.htm#RCMRF1948
