Try Database Modeling and Design by Teorey et al (http://www.amazon.com/Database-Modeling-Design-Fifth-Edition/dp/0123820200/). There's a Kindle edition available. If you don't have a Kindle, you can use the Kindle app for Windows (http://www.amazon.com/gp/feature.html?ie=UTF8&docId=1000426311).
