A few things I have found in the past, which may or may not be related:
- root.sh must never be run on box 2 until it has completely finished on box 1
- networking must be 100% consistent: eg: eth0 always public, eth1
always private
- if eth1 is public on one, and eth0 is public on other, blotto
- public and all VIPs resolved consistently - listed on DNS or in ALL /etc/hosts
- all userids must have no-password remote logins
- SSH must NOT have a 'You are in breach if you login' message
