Install RAC failed at running root.sh step

Hi all,
 

I am installing 11.2.0.3 grid infrastructure on RHEL 6 Virtual server and running into the following issue. Run root.sh failed at the end  - failed to start nodeapps. Have been working with Oracle support and the issue still has not been resolved.
 

The following is the error from running root.sh
 

PRCR-1086 : resource ora.cvu is already registered /u01/app/grid/bin/srvctl start nodeapps -n rhelsandbox ... failed FirstNode configuration failed at /u01/app/grid/crs/install/crsconfig_lib.pm line 9196. /u01/app/grid/perl/bin/perl -I/u01/app/grid/perl/lib -I/u01/app/grid/crs/install /u01/app/grid/crs/install/rootcrs.pl execution failed Tried to start nodeapps and failed with the following error:
 

[root_at_rhelsandbox bin]# ./srvctl start nodeapps -n rhelsandbox

PRKO-2421 : Network resource is already started on node(s): rhelsandbox
PRKO-2420 : VIP is already started on node(s): rhelsandbox
PRCR-1013 : Failed to start resource ora.ons
PRCR-1064 : Failed to start resource ora.ons on node rhelsandbox

CRS-5016: Process "/u01/app/grid/opmn/bin/onsctli" spawned by agent "/u01/app/grid/bin/oraagent.bin" for action "start" failed: details at "(:CLSN00010:)" in "/u01/app/grid/log/rhelsandbox/agent/crsd/oraagent_grid/oraagent_grid.log" CRS-2674: Start of 'ora.ons' on 'rhelsandbox' failed Could any one shed light on this?
