Take a look at note 1369107.1. I had the same issue when I installed 11.2.0.3 on a OEL6 with recent UEK kernel, after installation of 11.2.0.3 ACFS drivers were not installed (kernel not supported) and after patching with 11.2.0.3.3 the ACFS drivers were not started again after reboot. The solution was to run:

b) Run 'acfsroot install' on each node of the cluster. c) Run 'acfsroot enable' on each node of the cluster to restore the ACFS CRS resources.

to create the needed ACFS resources in CRS.
