Some of the stupid things I have done in years gone by, only once I might add!

Don't manually NID a 9i DB while it's open and running. Always fuser a DB file, before you rm it. Always check the full session info of that process you are about to "kill session".
Check the SID on that DB before you issue shutdown command. ( Once killed a 700 user prod DB once and on monthly reporting day just make sure the effect was complete! )

Check the server hostname before you issue "rm -rf *"

An ex-colleague once rm -rf the test DB at 2am, well he thought is was test...

It was closely followed by lots of frantic calls and a final trip to HR.
