To clearly show the relationship from the Oracle layer (ASM/raw), Linux, SAN. Get the following info

ASM:
-Label
-Path

Linux:

- Major Number
- Minor Number
- Device Name
- EMC Power Device Name (let's say you're on EMC)

SAN
- Logical Device ID
- Logical Device Name

Put these info in an Excel file. Any changes that you do, you must coordinate it with your storage admin or vice versa because you  don't want to accidentally format a device allocated for an ASM disk group... :)

You could look at a sample table on Part 1: 2b of this doc http://karlarao.files.wordpress.com/2009/04/single-instance-and-rac-kernel-and-os-upgrade.pdf
