Thanks Chris, very helpful as usual! I suspected that if there was any truth to this, it would be related to the mysterious new prefetching feature (introduced in 9.2 I believe, but still not documented). Any chance you could share your CREATE statements for the test case tables & indexes so I can play around with it? I'd like to see if setting "_table_lookup_prefetch_size"=0 eliminates the "db file scattered read"s and what kind of difference it makes on performance.
