You could be hitting a correct block or even a bad sector on a disk.    

  Try to rebuild to another tablespace....

Oracle SE 9.2.0.8, regular multi-column index. Apparently no index rebuilds done, indexes are validated every now and then. The corruption has hit this same index twice. No error messages anywhere, the query using the index just does not return any data.
