A long time ago (beginning 2001) I wrote a paper entitled "When should and index be used?" discussing such stuff. The link is: http://www.trivadis.com/Images/E_IndexStats_tcm17-7262.pdf

A couple of things on it, especially about DB_FILE_MULTIBLOCK_READ_COUNT, should be reviewed... but the rest, if I correctly remember, should be fine.
