Maybe the reason why you couldn't remove the SHM segment even with ipcrm, was that there were still some processes attached to it. This has happened to me on HP-UX few times.

NATTCH column in ipcs -ma output shows the number of processes attached to a segment. And LPID column may give some indication about the process to be killed.
