Check the log files(like Apache error.log, Jserv *jserv.log(s) etc.) under:

<IAS_ORACLE_HOME>/Apache/Apache/logs/
<IAS_ORACLE_HOME>/Apache/Jserv/logs/
<IAS_ORACLE_HOME>/Apache/Jserv/logs/jvm

Check the AOL/J Setup Tests:

http://<host:port>/OA_HTML/jsp/fnd/aoljtest.jsp
