I think oracle support just figured it out. I'll know when I try the install one more time. But this is what they said...

Although we have very little information in the stack from the core, I think I found the issue and it will be easy for you to test.

1) Please find the httpd.conf in the apache/conf directory.
2) Make a backup copy
3) edit the file and comment out or remove the one line LoadModule
perl_module libexec/libperl.so
4) Issue the following command
/u01/app/oracle/product/ias904/opmn/bin/opmnctl startproc type=3Dohs

I believe you have encountered a documented bug associated with a missmatch in perl libraries.

Once I did this, the HTTP server started. The server is a Sun E450/Solaris 8 and I'm installing Oracle AS10g (9.0.4) Infrastructure.  So it had nothing to do with how the server was cleaned up before the installation.
