I have first hand witnesses who attended Hotsos courses and they were both delighted. The other good place, according to the book, would be http://www.singingsql.com. It's owned and maintained by Mr. Dan Tow, author of a very interesting tuning methodology based on graph theory. The book he wrote is excellent. Dan's book focuses on finding an optimal execution plan, not application end-to-end tuning like Cary's book. If his courses are anything like a book, then they must be very good. For a competent performance analyst (the job description of a "DBA" is rapidly falling out of grace, so we disguise ourselves as "performance analysts" or, better yet, as "performance experts". This gives us all the advantages of the DBA position, without having to worry about backup and recovery).
I also believe that Dan Tow is a member of this list.
