I'd be logging a Tar as well at this point as a brief search on metalink suggests a number of potential causes for the 12500 as well as those already mentioned.

  1. Oracle Trace enabled.
  2. Oversizing the SGA and running out of process memory for Oracle.exe
  3. misset ORACLE_HOME
  4. no permissions on Oracle home for the account that runs the service (normally this would be the local system account on NT.

etc etc .
