Sounds like "Scope Creep" and trying to hit a moving target. You won't avoid some of this stuff, but best practice says to define the scope, gather business rules and GET SIGNOFF (along with many other things). Managers love visuals, so build a gant chart and keep all of the revisions to it in "Living Technicolor". GL (been dere, done dat).
