We do a heartbeat at fixed intervals ... a heartbeat consists of

logging in
inserting sysdate into a table.
sleeping for 3 seconds.
performing current_sysdate-sysdate_from_table
rollback
exit with output from step 4.

If the output from step 4 is excessive (> 5 or 6 seconds), DBAs, Unix Admins automatically get paged. This gives a check for instance, listener too. Some might think it is a bit over the top, but we like it.
